Please Bring
- Driver
- Insurance Cards
- Picture ID/Driver’s License
- List of the name and dosage of all your medications (prescribed and over the counter)
- List of all allergies
- Translator if needed
Please Do Not
- Eat or Drink anything including water after midnight prior to your surgery
- Wear Jewelry or bring any valuable with you
- Wear make-up, especially eye make-up, lotions, creams or colognes
- Please call us with any questions or concerns you may have.
